Sick notes can be requested in writing either on paper or by using our online form. Please note there is a 48 hour turnaround for sick notes. Your note will either be sent to you electronically via text message/online message or a printout will be left behind reception.

Please ensure that you state the reason you think you need a sick note, the start date and how long you think you need it for, along with any other relevant details
Please note that clinical staff will NOT issue a continuation of a sick note during an appointment.
